New Hume Moreland Website launched
The Hume Moreland Primary Care Partnership (HMPCP) is an alliance of more than 30 health and community service providers operating within the Hume and Moreland local government areas. The main aim of this partnership is to improve the experience of primary health care service consumers and to deliver improved health and well being outcomes for the Hume and Moreland Communities. The major strategic directions of the Primary Care Partnerships are:
- Service coordination: providing a consistent approach to identifying consumer health and care needs and the collection and sharing of client information
- Integrated health promotion: agencies working collaboratively to address priority health and wellbeing issues
- Integrated chronic disease management: agencies working together to prevent and manage chronic disease
